How to Get Started:

  1. Clients, family, friends or health professionals contact 811 to be connected to a home care office for further information regarding the CDHCI program or Self Managed Care and request an assessment.
  2. AHS case managers will provide comprehensive client assessments and care planning to determine the support services and funding eligible to each client.
  3. Coulee Home Care provides the services that have been arranged by the client.
