How to Get Started:
- Clients, family, friends or health professionals contact 811 to be connected to a home care office for further information regarding the CDHCI program or Self Managed Care and request an assessment.
- AHS case managers will provide comprehensive client assessments and care planning to determine the support services and funding eligible to each client.
- Coulee Home Care provides the services that have been arranged by the client.
